Safety / OSHA Injuries Additional Data Fields

Human Resources - Data Entry - Safety / OSHA:
In the properties Tab, I would like to have a field added to be able to assign a Case Manager to each injury.  We assign a person to follow a case through completion and would like to be able to easily see who has been assigned. 
Also, under the Treatments tab, I would like to be able to add additional treatment facilities.  One person may go to a clinic but then be referred on to a specialist and also have physical therapy throughout their injury.  With being to add multiple facilities, we would have a record of all facilities involved in a case.
