AR - Assign sales tax code at phase level (for T+M billing)

We need to be able to assign different sales tax codes to phases that represent work in different locations. Currently, we can only generate T&M bills with the default sales tax code setup in the T&M Job Billing Setup screen. This means we have to create a single bill for each phase after we change the sales tax code in the setup screen each time we select a phase onto a bill.
